My name is incorrect on certificate. How to request a change of my name?

Please email certification@forustraining.ie outlining that you wish to change the name on the certificate and include a copy of your photo ID (e.g. Passport, PPSN card or Drivers Licence) which has the exact name you want on your certificate.
